UPDATE: ASI FITNESS AND WEIGHT LOSS REIMBURSEMENT PROGRAM

❖ Fitness Reimbursement
Reimbursement of 50% toward the cost of fitness facilities and/or a series of fitness-related classes, up to $300 per year.

Qualifies –

  • Recognized Fitness Centers designed for health and fitness activity (examples: Bally, LA Fitness, YMCA, 24-Hour Fitness, Curves, and similar entities)

Does Not Qualify –

  • Activity fees, such as tennis court fees, bowling lane charges, fishing licenses, golfing greens fees, ski lift passes, boating slip fees
  • One-time event fees (e.g. entry fees for races for running, biking, walking or other sporting events)
  • Club dues (e.g. golf, tennis, wrestling)
  • Gym Equipment, gym clothing

❖ ASI Weight-Loss Program Reimbursement

  • ASI employees may submit for reimbursement of 50% of the amount paid for enrollment fees only (food not included), up to $300 annually, for a weight loss programs including Jenny Craig, Weight Watchers, and Profile. Other weight loss programs will need to be reviewed by the Benefit Council for consideration before reimbursement can be made.
  • All other fitness reimbursement guidelines and requirements apply.

Below are specific guidelines for the ASI Fitness & Weight-Loss Program Reimbursement –

  • You must be an active ASI employee at the time you paid your membership fees and at the time your reimbursement is paid.
  • Family gym memberships (not Weight-loss programs) are included if the employee is a beneficiary of the membership and intends to take advantage of the facility to address cardiovascular health.
  • Employees must submit applications quarterly, during the first month of the current quarter, for fees paid the previous quarter (example: Jan-Mar will be submitted in Apr).
  • Applications will be processed through ASI payroll and is subject to state and federal tax.
  • Reimbursements will provide 50% of the amount paid for Fitness and Weight-loss memberships and joining fees up to a maximum annual reimbursement of $300.
