Prescription EyeWear Program

Safety glasses or approved prescription safety glasses meeting the minimum requirements of ANSI Standard Z87-2 are required in all production areas of the factory. The company will provide one pair of prescription safety glasses per calendar year up to $96.00 with the approval of your leader and EHS. Please be sure to review your area’s PPE Assessment or job title description making sure to get the appropriate type (foamed lined vs non-foam lined).
Instructions for use:
1. Contact Ginger Akery to confirm the employee’s annual eligibility
2. Use the link above to generate the form (Link can also be found on RCMenu, Safety, Safety Glasses Request)
3. Add employee’s first and last name, SSO # and phone #
4. Print form
5. Employee’s lead signs the Supervisor Signature line (found near the top)
6. Employee takes signed form to Dr. Helwig’s office at 1581 N Main St, LaFayette, GA 30728.
Note: At this time, Dr. Helwig’s office is the only location for employees getting Roper funded prescription safety glasses.
